Cathedral. In the upstairs area beyond all of the laser-sight puzzle rooms. The Madonna is located on a pedestal blocked by metal bars. To release it you need to ring all five of the bells in their respective towers.
Madonna of Grief (Resident Evil 6)

This object is used in Leon's Chapter 2 inside the Cathedral. Place this in conjunction with the Madonna of Sorrow on the blue pedestals on the edge of the third-floor cathedral balcony overlooking the room to reveal a secret stairway on the lower level behind the dais.
